Tournament Liability Waiver Agreement

By signing your name below as participant, and/or parent/legal guardian of participant in Five Animals Kung Fu’s Skills Day Championship, you acknowledge that participating in the tournament exposes you to possibility of injury. You, being fully aware that participation exposes you to a possibility of personal injury. You, hereby release Five Animal Kung Fu Inc. and it’s organizing committee from any liability from any physical injury, and any other claims arising from or in connection with your participation in the tournament including claims that are known or unknown, foreseen, or unforeseen, future or contingent.

You acknowledge you have fully read and understand the Liability Waiver. This agreement shall be binding on you, and you, and your guardian or any legal representative.
